2012 Silver Stars Credential Application

Please complete this form to apply for Silver Stars season and single-game credentials for the 2012 season. Applications for season credentials must be submitted prior to May 14, 2012. Applications for single-game credentials should be completed no later than 48 hours in advance of tip-off or by 5 p.m. on Thursdays for Saturday and Sunday games. All requests will be reviewed by the Silver Stars Public Relations Staff and media will be notified if they have been approved for credentials no later than 24 hours before tip-off.
