American Fidelity Assurance Company PE Teacher/Coach of the Game Nomination Form

Parents and Students! Nominate your favorite PE teacher or coach to be one of seventeen teachers honored during a 2013 Tulsa Shock game! Each American Fidelity Assurance Company (AFA) PE Teacher or Coach of the Game is automatically in the running to win AFA PE Teacher or Coach of the Year! The person chosen as AFA PE Teacher or Coach of the Year will receive an additional $500 certificate for a class makeover or new fitness equipment and two season tickets for the 2014 Tulsa Shock season. In addition, each recognized Teacher or Coach receives:

• Two VIP tickets with food vouchers and one free parking spot to the game in which they receive their award.

• Thirty (30) individual tickets for the game they are receiving on-court recognition to invite students and/or family.

• During halftime, the PE teacher or coach will be honored with an on-court ceremony.

• A gift bag, including a desk-top memento, a $75 shopping card and a Tulsa Shock t-shirt.

• A photo of the teacher during the on-court ceremony, which will also be featured on the AFA PE Teacher of the Game gallery on the Shock website.

• A photo of the teacher during the on-court ceremony, which will also be featured on the AFA Teacher of the Game gallery on the Shock website.

• An invitation for two to attend the AFA PE Teachers and Coaches reception. This will be a pre-game reception before the Shock’s September 12th game against the Seattle Storm.
